1. Accidents involving injury should be reported to the employee’s manager immediately.
2. Someone who is properly trained and certified in first aid and CPR will assist the injured person.
3. If the employee refuses medical treatment, a Voluntary Refusal of Medical Treatment form should be filled out. (The form is located in Microsoft Outlook/ Public/ MBI Forms).
An English and Spanish version of the form is available. On the form, there is space for the manager to write a detailed description of the incident. Both the employee and manager will sign the document. Employee should also initial and read over the description.
4. If the employee requires medical treatment, the manager will contact Shannon Humphrey for further details and she will file all claims.
For Life Threatening Injury:
1. CALL 911 IMMEDIATELY!!! Someone who is properly trained and certified in first aid and CPR will assist the injured person , do not try to move the employee unless there is a chance the employee or others are in danger.
2. Notify managment as soon as possible.
3. Manager will then notify the appropriate personnel.
